JACKSONVILLE, Fla. – A woman turned herself in Wednesday on DUI charges in connection to a fatal crash that happened in July, according to the St. Johns County Sheriff's Office.
Deputies said Courtney Stack, 22, of Jacksonville, was charged with DUI causing a death of a human being and DUI.

The incident happened after 8:30 a.m. July 8 when Henry Smith, 54, was walking north on U.S. 1 North, just south of Lewis Speedway, deputies said.
The investigation revealed that Smith was walking in the grass on the west side of the roadway. Deputies said Stack was driving south in a 2013 Honda Civic and struck Smith.
Evidence at the scene, as well as witnesses to the crash, revealed Stack drove completely off the roadway before she struck Smith. Deputies said he was pronounced dead at Flagler Hospital a short time later.
Blood work that was obtained from the driver that day indicated there was cocaine and methadone in her system, according to the Sheriff's Office.
Detectives obtained an arrest warrant March 21 for Stack, who was found in Jacksonville with the help of the U.S. Marshal’s Fugitive Task Force, where she then turned herself in on the charges, deputies said.
Stack was released after posting $51,000 bond, deputies said.
