Nassau County is ranked near the middle of the pack -- 27th -- and Duval County comes in at 44th. Baker, Putnam and Union counties finish at the bottom of the rankings -- 63rd, 65th and 67th respectively.

"We are fortunate in St. Johns County to have excellent partnerships within the community including the Health Department, Flagler Hospital, the school district, local non-profit organizations and many others," said Mark Miner, chairman of the St. Johns County Board of Commissioners.

The rankings take into consideration issues like exercise, smoking and obesity, sexually-transmitted diseases access to health care and unemployment, even the number of fast food restaurants in each county, along with options for healthier fare.

For health officials, it's a gauge to determine where programs are needed.

"The county health rankings show us that much of what influences our health happens outside of the doctor's office," said Risa Lavizzo-Mourey, president and CEO of the Robert Wood Johnson Foundation. "In fact, where we live, learn, work and play has a big role in determining how healthy we are and how long we live."

In southeast Georgia, Camden County ranked the healthiest, coming in at 25th out of 156 counties in the study. Glynn County came in at 63rd, Charlton at 110th, Ware at 133rd and Brantley at 140th.
