JACKSONVILLE, Fla. – Florida Highway Patrol is investigating what caused a Ford Explorer to travel into the median on I-95 southbound at County Road 208 in Yulee, causing a chain reaction that sent the vehicle crashing into a semi-tractor-trailer truck.

A crash report from FHP said the SUV collided with a guardrail in the median redirecting the vehicle toward the right shoulder, where it collided with a semi-tractor-trailer truck entering the ramp to the Florida Department of Transportation Weigh Station.

The driver, a 48-year-old woman, and the passenger, a 67-year-old woman, were ejected from the SUV. The 67-year-old died at the scene.

Two girls, an 8-year-old and a 10-year-old were seriously injured, but not ejected, according to FHP.

The driver of the tractor-trailer was not injured. The investigation is ongoing. The crash report showed all involved were wearing seatbelts except for the 47-year-old driver of the SUV.
