JACKSONVILLE, Fla. – Investigators with Florida Highway Patrol are asking people who may have witnessed a hit and run involving a Putnam County School Bus to come forward. The crash report shows the school bus rear-ended a sedan on State Road 100 near Orange Park Avenue when the driver of the sedan came into the bus’ lane. The school bus driver was not able to determine the race or gender of the driver nor the make and model of the car. An 18-year-old and 12-year-old were on the bus at the time of the crash Monday afternoon. A 68-year-old man was attempting to pass another vehicle on State Road 100 in Union County Tuesday morning when his car hit standing water and hydroplaned, according to the Florida Highway Patrol. The vehicle left the road and the driver’s side slammed into a tree. The Lake City man died at the scene, troopers said. A 64-year-old man riding in the car suffered minor injuries, according to a crash report. Troopers said both men were wearing seat belts.

PALATKA, Fla. – A man was killed and another man was injured in a head-on crash Wednesday morning near Palatka, the Florida Highway Patrol said. The crash happened shortly after 4 a.m. on State Road 100 near Carraway Church Road. According to the Highway Patrol, a pickup truck was traveling west on S.R. The Highway Patrol said the tractor-trailer overturned, blocking both lanes of S.R. According to FHP, the driver of the tractor-trailer, a 59-year-old Alachua man, suffered minor injuries.

Troopers said there were reports of a single-engine gyrocopter that went down Saturday night. On Sunday morning, according to the Highway Patrol, crews found the crash site in the area of State Road 100, north of Pinecrest Circle. Troopers said the pilot was found dead.

PUTNAM COUNTY, Fla. – A 47-year-old man from Melrose has lost his life after a three-car wreck on State Road 100 near the intersection of Bardin Road in Putnam County. Crash investigators with Florida Highway Patrol said a 59-year-old woman from Palatka and a 72-year-old woman from Florahome were traveling west at State Road 100 when the driver lost control. The vehicle swerved right hitting a guardrail and then swerved left hitting an oncoming vehicle traveling eastbound, head-on. A 46-year-old female passenger, also of Melrose, is critically injured. A third vehicle was involved in the crash when it hit the back of the vehicle carrying the Melrose passengers.

An East Palatka man was killed in Wednesday in a head-on crash involving a box truck in Putnam County. The deadly two-vehicle wreck occurred shortly before 9 a.m. Wednesday along a stretch of State Road 100 east of East End Road, according to a Florida Highway Patrol crash report. Troopers said the 52-year-old man was behind the wheel of a sedan heading east on SR 100 when it crossed over the center line into the westbound lane and struck a box truck. The box truck veered off the road before the driver corrected and brought the vehicle to a stop on the shoulder where it caught fire, troopers said. The other driver had minor injuries.

PUTNAM COUNTY, Fla. – Three of four people hurt when a vehicle left State Road 100 and struck a tree Tuesday morning were airlifted to area hospitals, according to the Putnam County Sheriff’s Office. BUNNELL, Fla. – Three people died when their cars exploded in flames after a head-on collision Friday night on State Road 100. The three occupants in the two vehicles were pronounced dead at the scene near Bunnell, according to the Florida Highway Patrol. Authorities are investigating why one vehicle, which they believe was a pickup truck traveling at a high rate of speed, crossed the center line about 9 p.m. and struck a small passenger car head-on. The fire was so intense that the Daytona Beach News-Journal reports that investigators had difficulty identifying the makes and models of the vehicles involved in the crash. The victims were not identified as of late Saturday.

A Satsuma woman was killed and two others were hurt Sunday night in a single-vehicle crash in Putnam County. The wreck happened shortly before 10:45 p.m. Sunday near where State Road 100 meets Tram Road, according to a Florida Highway Patrol crash report. Troopers said a Ford Mustang carrying all three people was heading west on SR 100 when the car veered onto the shoulder and collided with a tree. One of the passengers, 43-year-old Mildred Bastian of Satsuma, died of her injuries. No one in the car was wearing a seatbelt, troopers said.

JACKSONVILLE, Fla. – A 44-year-old man has died after a crash on State Road 100 at South Pine Avenue in Putnam County. The Florida Highway Patrol said Peter Malave was traveling east on State Road 100 when his Dodge Ram truck veered off the road. The truck continued into the grass shoulder, hitting a cable box and then a wooden power pole. The vehicle flipped multiple times before stopping at the tree line near the roadway. Malave was taken to Putnam Community Medical but died from his injuries.

BRADFORD COUNTY, Fla. - A 3-year-old boy and a woman from Starke died Sunday in a head-on collision in Bradford County, according to the Florida Highway Patrol. The two-car crash took place on State Road 100 at Southeast 14th Avenue around 6 a.m. Sunday. Troopers said Shelby Rosenthal, 29, and Zander Rosenthal, 3, were killed when the Ford minivan they were in crossed the centerline and crashed into a Dodge pickup truck. A 6-year-old boy in the minivan was also seriously injured in the crash, troopers said. The driver of the Dodge Ram had minor injuries from the crash.

PUTNAM COUNTY, Fla. - A Putnam County man was killed and a Clay County woman was seriously injured in a wrong-way crash Thursday afternoon just northeast of Palatka, the Florida Highway Patrol said. The crash happened about 3:15 p.m. on State Road 100 near County Road 309D/Bardin Road. According to the Highway Patrol, a Jeep Cherokee was traveling west in the eastbound lane of S.R. According to the Highway Patrol report, he was not wearing a seat belt. The Highway Patrol continues to investigate.

STARKE, Fla. - A man, a woman and her 16-year-old son were killed early Tuesday morning in a head-on collision near Starke, according to the Bradford County Sheriff's Office. Deputies said a full-size pickup truck going the wrong way on State Road 100 at Southeast 109th Street just before dawn ran into a small car. Driver Laura Wilbert, 35, and her son, Christian Okrasinski, 16, both of Starke, were also killed, troopers said. The truck went airborne, flipped over and landed right where you're at right now," said a witness who didn't want his name used. All that's left behind is a bent street sign marking the intersection and deep tire tracks in the grass.

UNION COUNTY, Fla. – A 27-year-old Lake Butler man was killed and two others were injured in a two-vehicle crash Wednesday morning in Union County, the Florida Highway Patrol said. The driver of the Honda, Curtis Oliver Waters Jr., died at the scene, troopers said. The passenger in the Honda, a 54-year-old Lake Butler man, was transported to UF Health Shands Hospital in Gainesville with critical injuries, troopers said. The driver of the Ford, a 41-year-old Lake Butler woman, was also taken to the hospital with minor injuries, troopers said. Charges are pending, according to the Highway Patrol.

STARKE, Fla. – A 46-year-old man died and his two passengers were seriously injured in a single-truck crash on State Road 100 at Southeast 75th Street in Starke, the Florida Highway Patrol said. Both directions of SR 100 were closed at 8:36 a.m. after Andrew Garnett, of Keystone Heights, flipped his Ford Ranger, troopers said. It’s unknown why Garnett drove off the road. Garnett’s passengers -- Angela Lloyd, 42, of Melrose, and Robert Lloyd, 20, of Keystone Heights -- were taken to UF Shands with serious injuries, troopers said.

FLAGLER COUNTY, Fla. – A chemical company from central Florida lost its load in Flagler Beach Thursday morning, spilling 300 gallons of a sulfuric acid solution and closing a portion of State Road A1A. Hazmat crews neutralized the sulfuric acid solution spill by 2:30 p.m., and the Florida Department of Transportation reopened a portion of A1A by 5:30 p.m., Flagler County officials said. We don’t know how long all of that will take," Flagler County Public Safety Emergency Manager Steve Garten said Thursday. The Flagler Beach Public Works Department had the shoulder barricaded Friday as TME was still on site, said Flagler Beach City Manager Larry Newsom. Flagler Beach police are investigating the spill and charges against the driver of the truck are pending.

A 53-year-old Lake City woman died early Thursday morning when her car crossed into oncoming traffic on State Road 100 and struck a pickup truck head-on, according to the Florida Highway Patrol. Troopers said Cynthia Brandt was westbound about 6:35 a.m. when her Pontiac crossed the center line into the eastbound lane. An eastbound Ford Ranger driven by Lynn Jones, 47, of Lake City, swerved to the right, but couldn't avoid being struck by the car. Brandt died at the scene and Jones was airlifted to UF Health in Gainesville with serious injuries. According to the FHP report, neither driver was wearing a seat belt.

BRADFORD COUNTY, Fla. – A 52-year-old Gainesville woman was killed in a crash in Bradford County on Monday afternoon, according to the Florida Highway Patrol. State troopers said Mary Donn was driving east on State Road 100 approaching Southeast 109th Street about 4:25 p.m. when a pickup truck heading westbound turned left into her path. She was not wearing a seat belt. Karen MacCaister, 50, a passenger in her car, was taken to a hospital with serious injuries. Phillip Berryhill, 77, the driver of the pickup, was also hospitalized with serious injuries.
